Miley unveiled her new black and grey design last night (5 November) by sharing it with her 15 million Twitter followers.
The large vintage tattoo shows the face and neck of Miley's grandmother, Loretta Finley. Captioning the photo, Miley said: "Because I am her favourite & she is mine."
Miley got the personal tattoo inked by famed celebrity tattoo artist, Kat Von D, on Monday night (4 November).
A snap posted to Twitter shows the 20-year-old, who is dressed in a black crop top and trousers, reclining on a table in a dark room. Meanwhile, Kat is seen focusing on Miley's right arm as she applies the needle.
It appears that Miley may have paid homage to Loretta as an early birthday present.
Posting a cute black and white photo to Twitter last night, Miley is seen cuddling up to her grandmother, who she affectionately calls Mammie.
She said: "Still just as beautiful. @thekatvond look who left the house today. Happy birthday Mammie."
Kat, who has tattooed the likes of Lady Gaga and One DIrection's Harry Styles, also posted photos showing the design process, and we must admit, it looks like a spitting image!
Captioning the photo on Instagram, she said: "Love, love, loved tattooing you last night, @mileycyrus! #miniportraittattooparty."
